Click on the "Get a Sample" button with the blue arrow by it, then click on the pink paper at the bottom that says "Click for a FREE Sample from Costco". You do not have to be a Costco member (entering your membership ID is optional). Please note that all questions on the second page are optional to answer, but still click on the "Submit" button at the bottom.
